What to Bring to Your Appointment

To ensure your form is completed without delays, please bring:

• A full bladder: urine testing is a requirement for MTO physicals (checks for glucose & protein).

• Driver’s License: A valid Ontario Driver’s License.

• Corrective Lenses: Your glasses or contacts (if required for driving).

• Medical History: A list of current medications and any recent specialist reports (for heart conditions or diabetes).

• The Form: We have MTO forms on-site, but you are welcome to bring yours if it was mailed to you.
